I hereby certify that I am the parent/guardian of the player being registered to the TCIFA National Academy Football Programs. If I cannot be reached, I give permission for emergency treatment, emergency transportation, hospitalization, anesthesia, or injection, and will be responsible for the bills. My authorization does not include major surgery, unless life-threatening, and only then when the medical opinion of two licensed physicians or dentists concur in that treatment. I, my heirs, executors and, administrators waive, release and forever discharge the TCIFA National Academy and its staff, coaches, coordinators, employees & volunteers from all rights claims for damages, injury or loss to person or property which may be sustained or occur during participation in football-related activities.

PLAYERS CODE OF CONDUCT

Players participating in the TCIFA National Academy programs are exercising a privilege afforded them by the TCIFA in pursuit of community, national, and international recognition and success for youth football players. These players must exhibit the maturity to be successful in this pursuit. Thus, the following guidelines and rules shall apply in all activities with the TCIFA National Academy.
1. Players are expected to conduct themselves at all times in a manner, which is in keeping with representing the TCIFA, and will not bring discredit upon the FA.
2. When travelling with the TCIFA each player is expected to dress appropriately as befits representing the FA as directed by the Coaching and Management Staff.
3. Respect for property of others, adherence to the rules and guidelines specified here or by the Coaching Staff and observance of Turks and Caicos Islands laws are required for participation in this program.
4. Irresponsible and disrespectful behavior is cause for dismissal from the program for the remainder of the term or a stipulated period decided by the FA.
5. Failure to comply with any and all tournament or team rules {curfew attendance, dress code, schedules, etc.) may be cause for disciplinary action. Persistent failure will be cause for dismissal from the program for the remainder of this program and could affect a player's future participation.
6. Negative comments during football training and games are strongly discouraged.
7. Fighting, use of abusive or obscene language, stealing, teasing are prohibited.
8. Players are required to treat with utmost respect the environs and properties of the TCIFA. Littering and acts
of vandalism are strictly abhorred; infractions will be treated very strongly and may result in dismissal or suspension from the program.
I will conduct myself in a manner that represents the facilities, other players, referees, and the Coaching, volunteer and Administrative staff of the TCIFA National Academy at all times; especially when I am participating in football and related activities. If dismissal from the program or an event occurs while I am travelling with the TCIFA National Academy, I may be sent home immediately at my parent's cost by whatever means is most convenient for to TCIFA Administrators. No reimbursement of any program fees will be made, and non shall be expected.
We the undersigned, have read, understand, and agree to abide by the above guidelines and rules. We also agree to accept fair actions taken for failure to abide by these guidelines and rules.

TCIFA'S Philosophy:
TCIFA grassroots football is based on certain beliefs and philosophies: FUN, participation and everyone plays. The TCIFA encourages having a healthy spirit, mind, and body, meaning the development of the whole person. The TCIFA also emphasizes fundamentals, teamwork, fair play, cooperation, and respect for the officials one’s self, one’s team- mates, and the other team, the facilities and its environs. Our philosophy is simple: EVERYONE PLAYS – EVERYONE WINS. Every decision you make and every behavior you display is based first on what you think is best for your athletes, and second, on what can be accomplished to have everyone become a winner.
I hereby pledge to provide positive support, care, and encouragement for my child participating in the TCIFA National Academy football Programs by following this Parent Code of Conduct:
I will encourage good sportsmanship by demonstrating positive support for all participants, coaches, and officials. I will do my best to make this program fun for my child.
I will keep winning in perspective. A child usually forgets the outcome of the game: it is the adults that have a tendency to stress the win/loss record.
I will encourage my child/ward to treat other participants, coaches, officials, parents, and spectators with respect regardless, of race, age, sex, creed, or ability.
I will respect and treat with respect the facilities and environs of the TCIFA and encourage my child/ward to do likewise.
I will treat officials and coaches with respect. They are there to provide my child/ward with a safe and healthy learning environment. Officials and coaches deserve the same type of environment from the parents and spectators.
I will strive to be a good role model for my child/ward and the other children in the program. I will encourage my child/ward to learn to lose with dignity and win with grace.
I will respect my child’s/ward’s coach and do my best to have my child/ward at all practices and games on time. I will recognize the importance of volunteer coaches.
I realize they are important to the development of my child/ward and the program.
I will communicate with them and support their decisions.
